What Are Surgical Never Events?

When you have a surgical procedure performed, you hope that it will either improve your life or, in some cases, save it. Oftentimes, the result that is hope for is the one that is delivered. Unfortunately, this is not always the case. In fact, many patients are failed by their healthcare providers due to the occurrence of what is known as a “never event,” a term that was coined to describe serious incidents that should never happen and are entirely preventable. Never events can lead to death, amputations, lifelong conditions, and more, leaving patients worse off than before they sought medical treatment.

The list of never events is incredibly extensive. Never events include:

  • Wrong site surgery
  • Retained foreign objects
  • Wrongly prepared high-risk injectable medication
  • Misidentification of patients
  • Air embolism
  • Failure to monitor and respond to oxygen saturation
  • Maternal death due to post-partum hemorrhage

Due to the overwhelming occurrences of these never events, the World Health Organization launched the safe surgery checklist in 2008, modeled after the checklists used in aviation. The safe surgery checklist should be carried out prior to administration of anesthesia, before an incision is made, and before the patient leaves the operating room. When used, the checklist can significantly reduce never events, though it cannot compensate for the reckless or unprofessional behavior of a poor healthcare provider.

Additionally, when healthcare providers view the checklist as a tick-box exercise rather than as a valid tool to support better communication, failures can occur even when it is implemented. As long as healthcare providers are negligent or lax about a patient’s well-being and safety, never events will continue to happen.
